I paid $30 for my marriage license application with a $30 money order to the NYS Dept of Health through VitalChek on August 26th. The license was picked up on the 28th. I am very dissatisfied with your service and now request a refund of $30. I also had to pay extra for certified mail to ensure it reached you on time. Please refund my money promptly, or I will take legal action against your company. I have all my receipts and copies of the documents I sent on August 26th. Thank you.

I have been attempting to obtain my birth certificate for the last three months. I have been reaching out to VitalChek customer service, but unfortunately, when I finally got through to them, they were very rude and even hung up on me while I was trying to explain my situation. Now, my license has expired, and I urgently need my birth certificate. However, I am unable to proceed because of the lack of assistance. It seems like nobody is willing to help me resolve this issue promptly.

I am seeking a refund of $32.75 as I received incorrect information on my birth certificate. Despite informing the customer service representative of my name change decree when placing the order, I was twice informed that it wasn't necessary to provide the decree. The representative failed to mention that they would send exactly what was in the system. I explicitly requested not to receive another birth certificate with incorrect information. Throughout the conversation, I was never advised to contact the Virginia Health Department for my order due to the name change. After being on hold for two days for a total of 2 1/2 hours, I finally got through to Stephanie. She insisted it was my responsibility to know the correct procedure. I asked to speak with a supervisor, and she suggested I contact the Virginia Health Department. I mentioned that Vital check should handle the refund since they processed the order and failed to inform me of the correct procedure. Your assistance is appreciated. Thank you.

Regarding order no. [redacted]12, concerning the birth certificate request for Vaccaro Maria Cristina, submitted by attorney Sonia Ferrigno but incomplete due to missing documentation, an incorrect charge of €11.23 or $12.95 was issued. Thus, I kindly request the cancellation of order no. [redacted]12 and prompt reimbursement of the overcharged amount. I recently requested a birth certificate from VitalChek on January 13, [redacted]. I submitted all the required documents a week later as requested. However, every time I call their customer service line, I receive a pre-recorded message assuring me that my order has been processed, and I should receive my birth certificate from the D.C. office within 48 hours. This has been going on for five months now. When I check online, I am informed that my order status information has expired due to customer privacy protection. Despite attempting to contact customer service at 1-[redacted], I am only met with an answering machine. I have been diligently calling for days and weeks to no avail. I am eagerly anticipating the arrival of my Birth Certificate that I ordered from your company. I took a day off from work to ensure I am home to sign for the package. According to UPS's website, the delivery is expected today by 9 pm EST. After contacting UPS, I was informed that the order is pending from VitalCheck's end. I am concerned about missing another workday for the delivery. The tracking number is 1Z038Y[redacted]03. I need the Birth Certificate urgently for a DMV appointment to renew my expired Driver's License for driving and air travel purposes. On July 27, [redacted], I placed an online order for a certified copy of my birth certificate. It is now October 14, [redacted], and I have not received an order number, nor have I received the document. However, VitalChek deducted the fees from my bank account. I have attempted to contact VitalChek at great length with absolutely no success. On September 9, [redacted], I received a call from a VitalChek representative who advised me I would receive my certificate in 15 to 20 business days. It has now been 21 business days, and once again, I have not received the services I paid for. The circumstances surrounding this issue are affecting my employment. Specifically, I currently hold a national security clearance, and without the birth certificate, I cannot increase the level of said clearance. On Tuesday, October 13, [redacted], I filed a complaint with the Better Business Bureau in an attempt to resolve this issue. If I am unable to be made whole, my next course of action will be to file a class-action suit with the court.

On July 27, [redacted], I placed an online order for a birth certificate through VitalChek. Unfortunately, I have not yet received an email with an order number. On September 9, [redacted], I received a call from a VitalChek representative who informed me that I should expect the document in 15 to 20 business days. However, it has been 22 business days, and I have not received the birth certificate. I have lodged a complaint with the BBB, and if this issue is not resolved promptly, I plan to pursue a class action suit.

I placed two duplicate orders for Valentin Molina's birth certificate. 1st Order - [redacted]31 PIN - [redacted] 2nd Order - [redacted]13 PIN - [redacted] Valentin was very ill and on hospice when the first order was made with assistance. Vital Chek rejected his expired NM ID when proof of identity was requested and it was faxed to them. The second order was made by his daughter, Virginia Ortega, with the same request. This time the order went through, was processed, and we received the birth certificate promptly. Valentin Molina passed away on 10/17/20. I kindly request consideration to adjust the processing fee for the first order, #[redacted]31, as we are unable to provide a valid ID for him since his passing. Thank you for your understanding.

I am Máximo Ruiz Cárdenas from Mérida, Mexico. I have ordered to add my name as the father in my child CHANNING GRACE JUNEAU's birth certificate as per the 12th Judicial District Court's order (Docket Number 18-J9509). I have provided all required documents, including the court order. However, Vital Check is requesting unnecessary documents and the mother's signature. I have confirmed with the Amendments Department that these are not needed due to the court order. Please review the submitted order and send the necessary documents to the Amendments Department in Louisiana to add my name to the birth certificate. I would like to inquire about the status of a marriage certificate ordered on September 19 for the following individuals: - Essie H. (maiden name Beard) - Henry H. Unfortunately, we have not received it yet. After reaching out last week via Facebook messenger, where I was informed it was in Wayne County, we have not had any updates. I placed an order for my brother's death certificate on 8/10/[redacted] through VitalChek for $56.00. After not receiving it, I contacted customer service on 10/12/[redacted] and spoke with Jennifer about the delay. She promised to follow up but did not. Julie, the representative I spoke with on 10/23/[redacted], offered a refund and said she would call back on 11/02, but she also did not follow through. Cynthia on 11/03 explained the process and suggested contacting the funeral home. After confirming the paperwork was sent, I reached out to Cleveland Vital Records directly on 11/04. Unfortunately, after a long wait on the phone, I learned that VitalChek is not processing Ohio orders currently. I am seeking a refund of $56.00 for the unfulfilled service. Thank you.

I am having trouble submitting my phone number on the online order form for Illinois certified birth certificates. Even though I entered our valid landline number, the website keeps saying it's not valid, causing us to be unable to proceed with the order. I also tried to contact you through the "Ask a Question" feature on the FAQs page as advised on your site, but it requires an order number which we haven't obtained yet due to the issue we are facing. I am currently working on getting my birth certificate from Puerto Rico. However, I encountered a problem as my Massachusetts state ID has expired. I attempted to renew my ID using the birth certificate I possessed, only to find out that it is not considered valid as it was issued before July of [redacted]. I have my Social Security card, a college ID from Spring [redacted], and two expired Massachusetts state IDs from previous issuances. My fiancé needed to order his birth certificate from American Samoa since his ID expired and the DMV requires it for a new one. When his mom sent it, it was laminated, which is why we ordered a new one. However, he was denied because a government issued ID is needed, which we are trying to obtain.
